Home–Hospital Transportation Assistance

When you are undergoing cancer treatment, even the journey to the hospital can feel overwhelming. This scheme helps eligible patients travel safely between home and hospital for essential cancer care, so you can focus on your recovery with greater peace of mind.

What This Support Provides

Assistance for travel between your home and hospital for active cancer-related medical appointments:

Chemotherapy
Radiotherapy

Support is provided based on eligibility criteria and assessed financial need.

Eligibility

You may apply if you:

Are a Singapore Citizen or Permanent Resident
Have a confirmed cancer diagnosis
Require regular hospital visits for cancer treatment
Meet SCS’s financial assessment criteria

Each case is reviewed with care and consideration.

How to Apply

Step 1

Prepare the following documents required:

Copy of NRIC (front and back) of patient
Copy of Medifund, Blue/Orange CHAS, MFAC or PA card of patient
Copy of upcoming medical appointments schedule
Completed application form and endorsement by doctor, nurse or medical social worker
Copy of medical report(s) for Ambulance Service applications

Step 2

Approach your doctor, nurse or medical social worker to complete and submit your SCS Transport application form together with the required supporting documents.
